Why SMP Aftercare Determines Your Results

To understand why the aftercare rules exist, it helps to understand what just happened to your scalp.
During an SMP session, thousands of tiny pigment deposits are placed just below the surface of the skin, at the junction where the epidermis meets the dermis. Your skin immediately begins the healing process: the outermost epidermal layer starts to repair itself, gradually shedding the surface cells above the pigment.
This is why your results will appear to lighten between sessions. That gradual fade, commonly called healing fade, is not a sign that something went wrong. It is your skin doing exactly what skin does. The pigment beneath the outermost layer stays put. What the surface sheds is the skin cells immediately above it.
The final result lives in that stable dermal layer. But it only gets there cleanly if the healing process is protected.
Moisture, heat, sweat, sun, and friction all disrupt the epidermal healing process before the pigment can fully settle. That is why the first two weeks matter so much, and why the long-term habits that follow matter almost as much.
Follow the aftercare, and your SMP settles with sharp, natural-looking definition. Disrupt it, and the pigment can migrate, blur, or fade prematurely.

Phase 1: Pre-Treatment Preparation
Good aftercare starts before you arrive.
In the days leading up to each session, moisturizing your scalp consistently, once or twice daily with a fragrance-free, unscented cream or oil, helps soften the skin and allows the needle to create cleaner, sharper pigment impressions. This matters at every session, not just the first.
Avoid alcohol for 24 hours before your appointment. Alcohol thins the blood, increases bleeding during the procedure, and can dilute the pigment as it is placed, affecting the clarity of the result.
Eat before you come in. A stable blood sugar level reduces discomfort during the session and helps you stay relaxed throughout.
Arrive with a clean, dry scalp and no products applied. For men receiving hairline or full-scalp SMP: shave to approximately 0.5mm the night before using clippers or a trimmer. Do not use a razor blade on the day of your appointment. For women receiving density SMP: your hair remains at its natural length. No special preparation to your hair is required beyond arriving with a clean scalp.
Phase 2: The Per-Session Healing Window
This is the phase most clients think of when they hear "aftercare." It covers the 10–14 days following each session, and it applies every time you complete a session, not just after the first.
Days 1–4: Dry Care Only
Keep your scalp completely dry for the first four days. No washing, no shampooing, no sweating, no water contact of any kind. This includes rain. If you're commuting in Vancouver during the rainy season, keep an umbrella nearby during your healing window. Even a brief exposure during the dry-care phase is worth avoiding.
Do not go to the gym, sauna, or steam room. Do not touch your scalp with your hands. If you need to go outdoors, a clean, soft, loose-fitting hat provides protection from both light exposure and contact without irritating the healing skin.
The pigment will look darker than you expected. This is completely normal. Each dot appears larger and more defined immediately post-treatment; as healing progresses, the appearance softens into the result you are building toward. Do not pick, scratch, or disturb any scabs that form.
Days 5–7: First Gentle Cleanse
From day 5, you can introduce lukewarm water to your scalp. On days 6 and 7, a fragrance-free, sulphate-free cleanser may be used gently. Apply with your fingertips using the lightest pressure possible, then rinse and pat dry with a clean cloth or paper towel. Do not rub.
Begin moisturizing from day 5 with a light, unscented, non-comedogenic lotion. This helps the surface heal evenly and reduces the sensation of tightness or itching that is common at this stage.
Light activity, such as a short walk or gentle movement, is acceptable. Avoid anything that produces noticeable sweating.
Days 8–14: Settling Phase
Normal gentle cleansing continues. Your scalp may appear uneven or patchy during this period. Healing fade is normal and expected. The final result will not be visible until 6–8 weeks after your last session. Do not assess your results during this window.
Light to moderate exercise is acceptable, provided you rinse your scalp with cool water promptly after any sweating. Avoid chlorinated pools, hot tubs, and saunas.
Men: a trimmer or clipper may be used from around day 10. Hold off on razor blades until the skin feels fully settled and any residual sensitivity is gone.
Women: return to normal, gentle styling, but continue avoiding heat tools that contact the scalp directly.
Avoid hair and scalp products containing active ingredients, including minoxidil, glycolic acid, and strong dandruff-control shampoos, until two weeks post-session.
Phase 3: Between-Session Care
Most SMP treatments involve two to three sessions, typically spaced 7–14 days apart. The period between sessions is its own phase. It is not a pause in aftercare, but a continuation of it.
Between sessions, continue daily gentle cleansing and moisturizing. If your previous session's healing window has fully cleared, you can return to your normal exercise routine. Use SPF 30+ on your scalp whenever you are outdoors. Even on overcast Vancouver days, UV exposure contributes to cumulative pigment fading over time.
Avoid shaving until at least 10 days have passed since your last session. Your scalp may still be in the final stages of settling even when it appears healed on the surface.
During this period, your results will look lighter than the final outcome. Each session adds depth and density. The completed result takes shape only after the final session has fully healed, typically assessed at the 6–8 week mark.
For clients using topical minoxidil: pause application for one week after each session, then resume as normal. Minoxidil applied to freshly treated skin can interfere with the healing process in the treatment area.
Phase 4: Long-Term SMP Maintenance
Once your final session has healed and your results are assessed, the ongoing maintenance phase begins. This is less intensive than the healing window, but it is what determines whether your results last three years or closer to five.
Protect from UV exposure
Apply a broad-spectrum SPF 30 or higher to your scalp whenever you are outdoors, year-round, not just in summer. UV radiation is the single greatest cause of SMP pigment fading over time. Vancouver's summer months bring more direct sun exposure than many clients expect; carry a hat or apply sunscreen as a daily habit.
Moisturize daily
A light, fragrance-free lotion applied to the scalp after cleansing keeps the skin healthy and the pigment definition sharp. Dry, flaky skin accelerates the appearance of fading. This takes 30 seconds and has a meaningful cumulative effect.
Limit chlorine exposure.
If you swim regularly, rinse your scalp with fresh water immediately after exiting the pool. Prolonged chlorine exposure over months and years contributes to premature pigment breakdown. You do not need to stop swimming. Simply rinse promptly and reapply SPF before going back outdoors.
Plan for a touch-up.
SMP results typically last 3–5 years before a touch-up is recommended. This is not a failure of the original treatment. It is simply how semi-permanent pigment works over time, as skin naturally renews and pigment gradually lightens. Signs that a touch-up is appropriate: the overall tone has lightened noticeably from when it first settled, or the definition has softened to a degree that feels significant to you.
A Note for Women: Density SMP Aftercare

Most SMP aftercare guides are written with one client in mind: someone with a shaved head. For women receiving hair density SMP, the experience is different, and the guidance needs to reflect that.
At Integrated Ink, the majority of our female clients keep their hair at its natural length throughout the treatment and healing process. There is no shaving. The pigment is placed at the scalp level, between existing hair follicles, to create the appearance of greater density. Your existing hair remains exactly as it is.
This changes the practical experience of aftercare in several meaningful ways.
During the no-wash window (days 1–4):
Do not wash your hair or let water touch your scalp. If your hair genuinely requires attention, dry shampoo may be used from day 3 onward, applied to the lengths and ends only, not directly at the root or scalp. Avoid any product contact with the scalp itself during this window.
Styling during healing:
Avoid heat tools that make direct contact with the scalp. If you use a hairdryer, use a diffuser attachment on a low or cool setting and hold it at a distance. Avoid tight hairstyles, such as braids or ponytails pulled tightly at the root, that create friction or tension at the scalp.
Hair colouring:
Wait a minimum of four weeks after your final session before colouring your hair. Chemical processes applied near the scalp can interfere with recently settled pigment.
Topical treatments:
If you use minoxidil or other topical hair-loss treatments, pause application for one week following each session, then resume as normal.
The good news: your existing hair acts as a natural buffer during healing. It provides gentle protection from direct sun contact and environmental friction that clients with fully shaved scalps do not have. The density SMP result builds gradually and beautifully, and your hair, throughout the process, is always there.
What to Avoid After Scalp Micropigmentation
